Preparing for your appointment
- Check if you have a qualifying condition below in "Do you qualify?"
- Send qualifying condition medical records to Dr. Krantz at: Heather Krantz, M.D.

- If you do not have records, fill out a Request for Medical Records and give it to your doctor. Please send only records pertinent to the medical condition for which you seek an OMMP card. The condition must be documented in the records you have sent.
- After the records are received, reviewed and approved, you will be notified and may make an appointment with Dr. Krantz.
- Please print and carefully fill out the OMMP Application Form, the top personal information portion of the Attending Physician's Statement, and the Medical History & Consent form. Please bring these with you to your appointment.
- At your visit, you will meet with Dr. Krantz and a brief non-invasive physical examination will be performed, including blood pressure and pulse and your medical history will be discussed.
- You will then be given a packet containing all the necessary forms to be submitted to the State of Oregon.
Do you qualify?
Attending Physician's may recommend the use of medical marijuana for the following chronic medical conditions:
- Cancer
- Glaucoma
- A degenerative or pervasive neurological condition
- HIV/AIDS
- Post-traumatic stress disorder (PTSD)
- A medical condition or treatment for a medical condition that produces one or more of the following:
- Cachexia (a weight-loss disease that can be caused by HIV or cancer)
- Severe pain
- Severe nausea
- Seizures, including but not limited to seizures caused by epilepsy
- Persistent muscle spasm, including but not limited to spasms caused by multiple sclerosis
What are the rules about cannabis for medical patients in Oregon?
Marijuana is legal in Oregon for both recreational and medical use. Please check the OMMP website for current rules.
In Oregon, the Oregon Medical Marijuana Program (OMMP) is administered by the Oregon Health Authority.

How much marijuana can I possess if I am a medical marijuana patient?
An Oregon medical marijuana patient (and their caregiver, if applicable) may possess up to 6 mature plants, which must be grown at a registered grow site address, and up to 24 ounces of usable marijuana.

Is there a limit on how much marijuana an OMMP patient or caregiver can possess or purchase from a dispensary?
Yes. A patient or caregiver may only possess or purchase from a dispensary in one day:

Yes. A patient or caregiver may only possess or purchase from a dispensary in one day:
- 24 ounces of usable marijuana;
- 16 ounces of a medical cannabinoid product in solid form;
- 72 ounces of a medical cannabinoid product in liquid form;
- 16 ounces of a cannabinoid concentrate whether sold alone or contained in an inhalant delivery system;
- Five grams of a cannabinoid extract whether sold alone or contained in an inhalant delivery system;
- Four immature marijuana plants; and
- 50 seeds.
How do I get the card?
- You must have a visit with a physician licensed in Oregon (M.D. or D.O.) who signs an Attending Physician's Statement, which states that the patient has a qualifying medical condition and that medical marijuana may mitigate the symptoms or effects of that condition. The visit must occur annually.
- You must fill out an OMMP Application according to the application instructions
- You must submit the Attending Physician's Statement, the OMMP Application, proof of Oregon residency, fee (see application instructions or below) and proof of any fee discount to:

Payment is by money order or check. Checks must be made payable to OHA/OMMP.
It is recommended that you send in your documents by certified mail and maintain copies for yourself.
It usually takes about 2 weeks to receive your card.
- You may also submit the application, required documentation, and fee ONLINE. Payment is by credit card. You will have a temporary card within 24 hours if you choose this method.
- Documentation and fee must be submitted within 90 days of the date on the Attending Physician's Statement or the state will deny your application and a second visit will be required.
How much does it cost?
- You must pay a fee to visit our physician to obtain an Attending Physician's Statement. Please see Rates to understand the cost of the visit.
- After you have your Attending Physician's Statement and your OMMP Application , these may then be submitted by mail or online and the appropriate fee to the State of Oregon must be paid.

Grow Site Registration Fee (to be paid by the grower)There is a $200 Grow Site Registration Fee if ANY of the following is true:
- The grow site address is NOT the patient's residence.
- The grower is NOT the patient on the application.
- The grow site has MORE than 12 mature medical marijuana plants.
- The grower will be transferring medical marijuana to a dispensary or processing site.
